Manager ll Case Management – Wisconsin Medicaid (PS51315)
Location: This position will require you to reside within commuting distance to the Waukesha, WI office. In the future, you may be required to work from the local office.
The Manager ll Case Management for Wisconsin Medicaid is responsible for managing a team of care managers engaged in assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care for our non-dual SSI population across the care continuum and ensuring member access to services appropriate to their health needs. Primary duties may include, but are not limited to:
Responsible for process improvement, quality assurance, and adherence to appropriate medical policy, care management best practices, relevant clinical standards, and member contract.
Assigns department workload/cases to care managers.
Oversees state’s quality and case management expectations for SSI membership related to engagement rates, service timelines and interventions, etc.
Assists in ensuring health plan goals related to Cost of Care, service timelines and interventions, and utilization of inpatient and ER levels of care are met.
Serves as mentor to lower leveled managers and as subject matter expert for other areas of the company and within the department.
Manages multiple functions requiring unique sets of knowledge or has significant fiscal accountability.
Develops and manages annual operating budget.
Analyzes reporting to ensure state requirements are met.
Leads internal annual audits.
Hires, trains, coaches, counsels, and evaluates performance of direct reports.
Requires a BA/BS in a health-related field; 5 years of clinical experience; 2 years of prior management experience; or any combination of education and experience, which would provide an equivalent background.
Current, unrestricted RN, LCSW LMHC, LICSW, LPC, LMFT, LMSW or license in the state of Wisconsin required.
Experience leading a team (direct of indirect reports.)
Experience with, and understanding of, case management, interventions, and social determinants of health.
