Care Coordinator

Hours: Monday through Friday, 8am - 5pm

Special Programs Care Coordinator located in California in any of the following locations: Glendale, Los Angeles, Sacramento, Rancho Cordova The Special Programs Care Coordinator is responsible for providing clinical and general administrative support for the Special Programs Team, and for coordinating the delivery of care for members receiving services through the Health Homes program, Medical Meals, or Palliative Care. The Care Coordinator will be responsible for processing referrals for members to receive meals, referral coordination, reporting assistance, materials management, and call/fax/e-mail triage. As Special Programs moves toward the implementation of Enhanced Care Management and In Lieu of Services as part of CalAIM, the Care Coordinator will manage the incoming applications and documentation submitted by community partners. The Care Coordinator will also serve as a liaison between the Special Programs team and internal/external stakeholders.

Primary duties may include, but are not limited to:

  • Triages incoming communication (telephonic and email) to evaluate the appropriate follow up.

  • Verifies program eligibility and identifies alternate levels of service or programs when applicable.

  • Supports clinical team and coordinates requests for medical records and other relevant information/documentation.

  • Coordinates provider and member communications, enrollment processes, and document management/oversight.

  • Maintains program enrollment and discharge records, along with accurate and complete documentation in Case Management database.

  • Coordinates member cases with clinical and interdisciplinary teams.

  • May manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities, to insure cost effective and efficient utilization of programs (does not involve clinical judgment).

  • Collaborates with departments and completes day-to-day activities related to program assignment, including data entry and correction of errors.

  • Additional responsibilities may include; scheduling, conducting and attending meetings, supporting management in overall objectives, assessing costs and savings and assisting in the payment process for vendor invoices.

Qualifications

Requirements:

  • Requires a high school diploma; 2 years of experience in Case Management Care Coordination in a Managed Care environment; 3 years of experience in a general office/administrative environment; or any combination of education and experience that would provide an equivalent background.

  • Medical Assistant Certification preferred.

  • BA/BS degree preferred

  • Experience in working with the Medi-Cal population, including but not limited to seniors, persons with mental or physical disabilities, or individuals with chronic medical conditions preferred

  • Must have excellent communication skills (written and verbal) and interpersonal skills

  • Must have strong PC skills (Word, Excel, Outlook)
